Opened as the first phase of The GreenFields Continuing Care Community, GreenField Health & Rehabilitation Center offers long-term skilled nursing, memory care, and outpatient rehabilitation in a supportive, faith-based environment.

The 120-bed facility was developed to replace Niagara Lutheran’s Delaware Home operations, providing residents with a modern care setting that supports independence and dignity.
